Requirements for Admission

Teays Valley Child Development Center is licensed to care for children between the ages of two and twelve who have at least begun the process of toilet training.

We do not discriminate against children on the basis of race, color, gender, or national origin.

Special Needs

We accept children with special physical or emotional needs, if the child's need can be accommodated. If we cannot guarantee a top quality experience that meets your child's needs, we will be glad to refer parents to other resources in our area. Please be sure to let us know of any special circumstances when you apply.

Day Care and Preschool Admission Packet

If you are interested in enrolling your child in the daycare or preschool program at Teays Valley Child Development Center, the following information must be completed.

  1. Application - This is the first step and will allow us to ensure that we will be a good fit for your family.
  2. Physician's Report - This report should detail your child's health status.
  3. Emergency Card - This card gives us detailed contact information in the unlikely event of an emergency. (Two copies)
  4. Food Reimbursement Eligibility Form - Daycare enrollment only!
  5. Payment Agreement
  6. Immunization Record
  7. Complaint and Grievance Form
  8. Policy Agreement

Children must be four (4) years old before September 1 of the school year to qualify for our preschool program. Please note, that families enrolling in both day care and preschool will only have to fill out the paperwork one time.

Enrollment Fee

When your application is approved, we require a non-refundable enrollment fee and a written report from your child's physician.

After School and Summer Program Admission

For admission into our after school and/or summer program, please submit a completed application,along with a physician's statement. Fee schedules for after school and summer program vary. Contact us for more information.

Preadmission Visit

We strongly recommend that parents and children visit the center at least one time before their first day. This visit helps the children make an easier transition.

During this visit, we will provide you with information about our policies and our program, including an overview of our curriculum. We will also answer any questions or concerns you may have about our center and our operations.

We welcome parent involvement in our children's education. While we do try to keep disruptions to a minimum during preschool instruction, we encourage parent visits. Parents are requested to use good judgment concerning their child's ability to adjust to their presence in the classroom and re-adjust when they leave.
